Entry Forms

IMPORTANT ENTRY FORM INFORMATION
Special attention should be given to the filling out of entry forms. Be sure that your name, address and city are clear on each form filed. Whenever signatures of affidavits are necessary, be sure they are supplied. Check your division and class numbers carefully with the Premium Book before listing them on the entry form. Wrong division or class numbers appearing on entry forms can lead to disqualification if not corrected in time. Separate entry forms must be used for each separate department. For example, an animal entered in the Beef Cattle Division can not be included on the same entry form as one used for Dairy Cattle, or preserved foods should not be listed on the same form as needlework. Particularly, we must call attention to the fact that agricultural and horticultural entries must not be combined. Whenever in doubt, do not hesitate to check with the office of the Fair Management (628-3421) to make sure that your entry is correct.
